This was made using Truly, Madly, Deeply by DarleneH and the kit can be found here:

I found a free heart topped box template on the internet, and used that with one of the papers from the kit. I also cut out several petals and used those to make the flower on the front, adding a leaf from my stash. Then to finish the box off, I used distress ink on all the box edges.
